In order to close your account, please complete the form below. This form must be completed along with any share withdrawal or share to loan forms that may be required to complete any loan or share accounts that you may have.

By completing this form, you understand that you are closing your account with Beacon Savings & Loans and will no longer have access to those services. You also acknowledge that you will lose your right to receive dividends and interest rebates should any be paid.
